    Radiologic Technologist Program Application for Admission

    Before beginning this application, please review the Radiologic Technology Program website.

    Please note: This application must be completed in one session. You will not be able to save your application and complete it later. This application may take between 15 - 30 minutes to complete.
    Applications are due by May 10th for August Admission Consideration.

    Application Requirements

    1. Completed Radiologic Technologist Program Application for Admission.
    2. TEAS Assessment Entrance Exam with a minimum composite score of proficient. The TEAS test must be dated within a year of application. There is a limit to the TEAS testing, two times per semester, and four times within a year. Study guides are available in the bookstore, online, or at the Nursing Education Center. 
    3. Background check with a valid DPS level one fingerprint clearance card.
    4. Background check processed through Corporate Screening Services, Inc.  (Use code EAST95EC)

    5. Physical Examination form completed by a licensed health care provider.
    6. A 250-500 word essay (double-spaced) describing why a career in healthcare is desirable to you. (Do not put your name on the essay.)

    Admission Requirements

    Documentation of admission requirements are due in the program director's office by 2 business days prior to the start of the semester or the applicant’s admission slot is forfeited.
    1. Meet admission requirements to Eastern Arizona College.
    2. Current American Heart Association CPR Basic Life Support (BLS) card.
    3. Complete Immunization Record and Radiologic Technologist Physical Form. *
    4. Urine drug screen through Corporate Screening Services, Inc. (use code EAST95EC)
    5. Level One Fingerprint Clearance Card.
    6. Transcript verification of the following prerequisites with grades of “C” or better:
         - CHM 130 or higher
         - MAT 154, MAT 181 or MAT 220
         - BIO 201 Anatomy & Physiology I
         - BIO 202 Anatomy & Physiology II
         - BUA 160 Medical Law and Ethics

    *Waivers may be available. EAC staff will assist students to work through the clinical partner to request waivers. Note that not all clinical partners have a defined process for requesting an exemption.

    Please Note:
    -Applicants with a healthcare related certificate or degree from an accredited institution can receive a point towards admission.
    -Applicants who complete all their pre-requisite courses at Eastern Arizona College will receive one bonus point towards admission.
